I’ll be setting you up manually within the next few hours (as soon as I get in front of the computer) to make sure you get the right questionnaire.
You will receive an email from PT Enhance which is the service I use for hosting the questionnaire.
You’ll receive a link, a username and a password.
As soon as you log in, you’ll have access to start filling out the questionnaire.
Once you complete it, you will have access to the results immediately.
The chart will tell you which areas are your highest priority so you can immediately take action to resolve it.
